We operate a hybrid care model that blends in-home in-person and virtual care that leverages unlimited video telemedicine, SMS text-based care, and an on-demand resource library that increases access and satisfaction for our members.

Care Circle Admission

At the time of a patient’s admission, we also begin providing support to their loved ones. Collectively, we refer to patients and their loved ones as a Care Circle, and we provide customized support to loved ones during hospice and in bereavement for as long as needed.

What services do you offer?

Guaranteed Hospice offers routine home care and continuous home care. For more acute levels of hospice care, we partner with trusted providers to offer general inpatient and respite care. Please reach out for partnership and revenue-sharing opportunities. 

What areas do you serve?

We serve patients and families in Southern California including the following counties: Los Angeles County and Orange County.

What insurances do you accept?

We currently accept Medicare, and we can review Medi-Cal and or other commercial insurance for coverage. We believe great end-of-life care is a healthcare right and we strive to never turn away a patient due to an inability to pay.

Who is your medical director?

Dr. Angela Tabibian is our medical director. Prior to joining our team, she worked at Kaiser Permanente-Woodland Hills Medical Center as a hospitalist for more than 12 years and served as the Physician In-Charge for Hospice, Home Health, Palliative Care, and Community Programs for three years. She is board-certified in Internal Medicine. She received her MD from the USC Keck School of Medicine and completed her Internal Medicine residency at UCLA.

Our agency is accredited by the Accreditation Commission for Health Care (ACHC). Our agency demonstrates a commitment to delivering the highest quality care and services by complying with ACHC standards and the Centers for Medicare & Medicaid Services Conditions of Participation.

How do you make it easy for us to work with you?

Treat us as an extension of your team. We maintain a high-touch relationship with your team to ensure regular updates on your patients through in-person or virtual meetings and weekly care summaries. We take on all coordination needs upon a patient’s referral.

What languages does your team speak?

Our team includes care providers fluent in English and Spanish. For all languages, we offer two-way translation services for every interaction.
