Our compassionate interdisciplinary team of providers and volunteers deliver care centered on dignity and quality of life. We will be here for you and your loved ones every step of the way.
Oversees your plan of care and provides direction to the hospice team
Responsible for assessments, care planning, monitoring your condition, and symptom and medication management
Help with personal care, hygiene, and activities of daily living
Facilitates onboarding to Guaranteed Guide and seamless communication of concerns and questions
Provides social support and coordinates services, education, and access to community resources
Offers spiritual or faith-based support to you and your loved ones

YOUR CARE CIRCLE
At Guaranteed, we care for you and your loved ones.
Collectively, we refer to patients and their loved ones including family, friends, and other caregivers as the Care Circle.
We’ll work with you and your care circle every step of the way to ensure you receive the best care at the end of your life.
LEVELS OF CARE
Guaranteed Hospice personalizes each care plan based on your and your family's needs. Medications, equipment, and supplies are provided throughout the entirety of your care.
Routine Home Care
Regular care visits to whatever location you consider home, ensuring symptoms and pain are managed and that you are comfortable
Continuous Care
Frequent support during critical episodes to manage acute symptoms until routine services can resume
Inpatient Care
Routine and continuous care that require you to live in a skilled facility like a hospital or nursing center
Respite Care
Short-term inpatient stays to allow the primary caregiver to rest while you receive care in a skilled facility

Hospice care is typically 100% covered by a patient's insurance provider. We believe excellent end-of-life care is a healthcare right; we strive to never turn away a patient due to an inability to pay.
Who qualifies for hospice?
We serve individuals who have a terminal diagnosis, have a life expectancy of six months or less, and are ready to stop curative treatments.
What areas do you serve?
We serve patients and families in Southern California including the following counties: Los Angeles County and Orange County. Stay tuned for service in Pennsylvania soon.
What is not covered by hospice care?
- Curative treatment and prescriptions for your primary diagnosis
- Caregiver
- Room and board
- Hospital, Emergency Room, or ambulance services
- 24-hour care
What are the signs I should consider hospice care?
